STEP 3
Step 3 exam eligibility periods are assigned immediately once your application has been processed and approved. Time for processing varies but is usually between 3-4 weeks. Upon complete processing of your Step 3 application and confirmation of eligibility, a Scheduling Permit will be issued to you with instructions for making an appointment at a Prometric Test Center. The Scheduling Permit specifies the eligibility period (beginning immediately and extending for approximately 90 calendar days) during which you must complete the examination. For further details regarding postponing or rescheduling your exam, please refer to the USMLE Bulletin of Information. Please note that Step 3 is a two-day test. The two days on which you take the test must be consecutive, unless the center is closed on the day that follows your first day of testing. If you are applying for the USMLE through New York State, you must submit a licensure application at the same time you apply for Step 3. Most states do not require simultaneous application for a medical license, but you should call 1-817-868-4041 to determine if your state has specific requirements prior to taking Step 3.

TRANSCRIPT REQUEST (ON-LINE)
In order to complete the process, your scores must be released to the state medical boards. This is performed through the Federation of State Medical Boards (FSMB) Transcript request (http://www.fsmb.org).

• **Note: Be sure to submit at least TWO recipients, as the cost is equivalent to 1. Even if you are thinking about possibly practicing in another state, select that state as the second recipient – it will save you money in the future.
NYS LICENSE
If you are applying for the USMLE through New York State, you are required to apply for a New York State Medical License concurrently. This results in paying for two large fees (USMLE and Medical Licensing) simultaneously. Some residents choose to apply for the USMLE through other states (CT, MI, NJ) in order to stagger the cost. In New York, you will not receive a USMLE Step 3 test date until you have completed your application for a New York State medical license which includes all of the forms below and the application fee. The New York Education Department will not warn you if you are missing a form. You must be sure to fill them all out and contact them continuously to ask if anything is missing.

• Section 2 to be completed by Professional School Attended (Registrar, Dean, Rector, or Principal of the medical school)
FORM 2PGT CERTIFICATION OF APPROVED POSTGRADUATE TRAINING
• Section 1 to be completed by Applicant
• Then submit form to Residency Director for Certification of Postgraduate Training
MANDATED TRAINING RELATED TO CHILD ABUSE
Effective January 1, 1989, Education Law required physicians, when applying initially for licensure to provide documentation of having completed two hours of coursework or training regarding the identification and reporting of child abuse and maltreatment. If you went to medical school in NYS, your training was integrated into your medical school curriculum.
